Influence of physiotherapy and regimen recommendations on lymphedema therapy

This bachelor thesis deals with the issue of lymphedema and the effect of physiotherapy and regimen recommendations on this type of swelling. In the theoretical part of my bachelor thesis, I describe in the anatomy of the lymphatic system in the first chapter. The second chapter describes the lymphedema, its classification, clinical picture, diagnostic methods and complications of lymphedema that may occur. Lymphedema therapy and regimen recommendations are also a very important subchapter. I elaborated the practical part in the form of qualitative research and subsequnet creation of case studies. I selected four patients with lymphedema, whom I monitored lower limb circumferences in connection with regimen recommendations and physiotherapy. These data were compared with the subjective feelings of patients and subsequently the research was evaluated. The main aim of this bachelor thesis was to present possible regimen recommendations in treatmen of lymphedema and to evaluate their influence on the course of edema. Research shows that even partial adherence to the proposed recommendations has the effect of reducing limb swelling.
Physiotherapy after breast surgery

The aim of this work is to describe the issue of breast cancer surgery and the role of physiotherapy in this area. The theoretical part describes basics of breast anatomy, defines breast carcinoma, its diagnosis, risk factors, types, ways of spreading and symptoms of the disease. For the prevention and treatment of postoperative complications is crucial early and long-term physiotherapy of the patients. Next chapter describes the objectives of physiotherapy, kinesiological analysis of patients and most frequently used therapeutic techniques. The last chapter deals with the psychological aspect of therapy and confirms the significant influence of psychic on the results of treatment and on the quality of life of the patients. The practical part aims to analyze the physical condition of the patients after breast surgery and to evaluate the effect of physiotherapy on the course of treatment and prevention of postoperative complications. Evaluation of postoperative lymphoedema after differently radical surgery for vulvar and cervical carcinoma

The aim of this study was a prospective detection of postoperative lymphedema of the lower limbs in patients after the surgery for cervical and vulvar cancer using different methods of examination and their comparison and monitoring of postoperative complications and quality of life. Totally 78 women were followed after the surgery for cervical cancer and 36 for carcinoma of the vulva. Due to the radicality of the surgery the patients were divided into the conservative and radical groups. Lower limbs lymphedema were evaluated preoperatively and 3, 6 and 12 months after the surgery by the measurement of the lower limbs circumference, multifrequency bioelectrical impedance analysis (MFBIA) and subjective feeling. Quality of life using the European Organisation for Research and Treatment of Cancer (EORTC) questionnaires was evaluated before and 6 and 12 month after the surgery. 12 months after the cervical cancer surgery 35.9 % of patients reported subjective lymphedema, 37.18 % lymphedema were objectively diagnosed by the measurement of lower limb circuits and in 52.56 % of cases the increase of amount of extracellular fluid was detected by the MFBIA Ri/R0 method. Possible ways how to affect lymphoedema after breast ablation in physiotherapy

This thessis focuses on breast carcinoma which is a serious illness. I am devoting to problematics of lymphedema resulting from breast ablation. The aim of theoretical part of this thessis is to describe the problematics of this illness. I am focusing on the treatment complications, especially lymphedema which is very common complication. Practical part of this thessis was realized on observing the lymphedema of patiens according to physical excercise and another possibilities of treatment. The metodics of practical part was based on qualitative research. The research was carried out on cooperation with three patiens who went through surgical treatment, radiotherapy and chemotherapy. All three patinets went througt treatment course two or three years ago and were selected considerng to surgery range. I used the methods of interview, anamnesis, kinesiology analyses and analysis of medical records. The results were processed into a case studies. The entrance examination data were acquired at the beggining of the observating and the final examination data were acguired three month later. The observating confirmed my hypothesis that physical excercising and following recommended regime leads to lymphedema reduction. This thessis can be used by thephysiotherapists, another healthcare workers and also by the pacients.
The role of nurse in the care of patients with lymphedema.

Current situation The title of this bachelor's thesis is: "The Role of the Nurse in the Care of Patients with Lymphedema". As the title indicates the topic is the role of the nurse if the care of patients suffering from lymphedema. Lymphedema is a condition when the lymphatic system retains the fluid in the body and the fluid causes lymphatic tissue swelling. The lymphedema can be inherited or caused by a birth defect, in such a case it is a primary lymphedema. When we talk about secondary lymphedema, it means the lymphedema was caused by lymphatic system malfunction. The causes of secondary lymphedema can be surgeries with inevitable interventions in the lymphatic system, most frequently interventions in sentinel nodes. Other interventions increasing the risk of lymphedema are radiotherapy, chemotherapy, iatrogenic lesions and/or accidents. The most frequent causes are surgical interventions, in particular in case of breast ablation and/or gynaecological cancer. Objective of the thesis Two objectives were selected for the research part of the bachelor's thesis: Mapping the role of the nurse in the care for patients with lymphedema from the nurse's perspective and Mapping the role of the nurse in the care for patients with lymphedema from the patient's perspective. Methodology In order to fulfil the first objective we decided to use the technique of qualitative research survey, in particular in-depth interviews. The research set consisted of six nurses from gynaecological and surgical departments of the Nemocnice Rudolfa a Stefanie Benešov, a.s. Hospital. In order to achieve the second objective we chose the same technique of qualitative research survey in the form of in-depth interviews. The research set consisted of six patients suffering from lymphedema. We carried out altogether fourteen interviews within the research between March and June 2017. All respondents came from the Central Bohemia Region near the town of Benešov. Upon the transcription of the interviews in Microsoft Word the data was classified and processed by open coding. Results Upon analysing the data four categories of results from the interviews with nurses appeared. The categories were: Nurses' knowledge of the problem, Regime measures, Specificities of the nursing care and Education of patients after a risk intervention. The results of the interviews with patients were classified into five categories: Occurrence of lymphedema, Treatment of lymphedema, Regime measures, Specificities of the nursing care and Education focused on prevention. Conclusion The objective of the bachelor's thesis was to map the role of the nurse in the care of patients with lymphedema from the nurse's as well as the patient's perspective and we also wanted to study in more detail the issue of lymphedema and specificities of care for patients suffering from this condition. The thesis focuses also on the education provided by the nurse regarding primary prevention. The analysis of the acquired data revealed deficiencies in the education and generally primary prevention, and therefore an information leaflet for nurses was prepared.

Lymphedema of the upper extremity as a complication after breast tumour curative radiotherapy.

Lymphedema of the upper extremity is a serious complication after breast cancer treatment. This thesis concerns lymphedema of the upper extremity after a curative treatment for breast cancer. Describes breast cancer therapy, its methods and curative purposes. Explain what sentinel node disection is and what compliactins expect after its removal. Becouse essential part in breast cancer treatment také radiotherapy, thesis intoduces radiation dozes, radiation volums and radiation therapy technique. The goal of this thesis was to evaluate number of lymphedema of the upper extremity at patients after axilary node disection and at patients after sentinel node disection and adjuvant radiation therapy treatment. The explorative question was whether a sentinel node disection reduces origin of lymphedema upper extremity independently on radiation therapy extent and iradiation volum. The metholodgy was based on collecting data of patients with breast cancer tumour from register of oncological patients in Hospital České Budějovice joint stock company. Data were subsequently processed and compared to data obtained from vocational literature.
